Commissioner of Oaths
The Council Services Department provides the services of a Commissioner of Oaths only on the specific documents listed below. The Commissioner of Oaths is a person who is appointed under the Commissioners of Affidavits Act to administer an oath for the purpose of declaring and affirming that the contents of a document are true.
This can be done on the fifth floor of Windsor City Hall, 350 City Hall Square West, Suite 530. The cost for commissioning a document is $30.00.

Notary Public
The Council Services Department does not offer notary services. Should you need a document certified, attested to be a true copy or notarized, you will need to contact a Notary Public or lawyer.
Authentications and Apostilles
Official Documents Services is the authority that issues certificates of authentication and apostilles in Ontario for a document to be used outside of Canada. This service is provided at the ServiceOntario office located on the second floor at 400 City Hall Square East, Suite 205. Fees may apply.

- Domestic and Foreign Pension documents (Proof of Life documents will specify which individuals are authorized to certify/confirm the document.)
- Immunization exemptions
- Statutory Declaration of Apprenticeship Hours
- Canadian Citizenship forms
- Delayed Statement of Live Birth
- Legal Name Change forms
- Affidavit of Lost Cheques
- Statutory Declaration of a Lost/Stolen/Counterfeit Bank Card
- Sworn Statement for the Transfer of a Used Motor Vehicle in the Province of Ontario
- Statutory Declaration for Ontario Student Assistance Program (OSAP) – Statement of Common Law Status/Supporting Children
- Travel Affidavit
- Statutory Declaration of Theft of GST or Federal Cheques
- Affidavit of Unregistered Vehicle (boats, motorcycles, snowmobiles, trailers etc.)
- Affidavit of Proof of Residency
- Affidavit for Lost/Stolen or Damaged Passport
- Affidavit of Proof of Loss of Motor Vehicle/boat (from an insurance company)
- Amendment to Birth/Death/Marriage Certificate forms
- Statutory Declaration by a person for a change of sex designation
- Passport in Lieu of Guarantor (specific federal government form only)
- Statutory Declaration by contractor regarding holdbacks/security deposit
- Statutory Declaration of Progress Payment Distribution – subcontractor
- Registered Retirement Savings Plan (RRSP) withdrawal due to financial hardship
- Travel Industry Council of Ontario – Affidavit of Customer Claimant
