Applicant Requirements:

Holistic Centre Application
Business Ownership Declaration
(2) pieces of government-issued identification including one photo I.D. as well as one I.D. demonstrating proof of status in Canada (e.g. birth certificate, Canadian passport, Canadian citizenship card, permanent resident card, work permit, etc.)
Certificate of Incorporation, if applicable
Master Business Licence, if applicable
Police Clearance (original, not more than 30 days old for new applications and not more than 365 days old for renewals)
Certificate(s) and/or diploma(s)
Verification of membership in a complementary health care organization
Code of ethics
Samples of intended advertising material
Certificate of insurance showing at least $2,000,000 minimum liability coverage
Floor plans (2 copies), new applications only
List of employees' legal names and valid municipal licence numbers
