Parlour Applicant Requirements:
- Application - Body Rub Parlour - Owner/Operator
- Business Ownership Declaration
- (2) pieces of government-issued identification including one photo I.D. as well as one I.D. demonstrating proof of status in Canada (e.g. birth certificate, Canadian passport, Canadian citizenship card, permanent resident card, work permit, etc.)
- Certificate of Incorporation, if applicable (new only)
- Master Business Licence, if applicable (new only)
- Proof of applicant's right to occupy premises (lease, deed, etc.)
- Police Records Check - original, issued within 30 days of application
- List of licensed attendants, including licence numbers
- Copy of proposed advertising
- (2) passport-sized colour photographs, taken within 30 days of application
- Establishment & Property Inspection Certificate from Health Unit - Visit www.wechealthunit.org.
Attendant Applicant Requirements:
- Application - Body Rub Parlour - Attendant
- (2) pieces of government-issued identification including one photo I.D. as well as one I.D. demonstrating proof of status in Canada (e.g. birth certificate, Canadian passport, Canadian citizenship card, permanent resident card, work permit, etc.)
- Police Records Check - original, issued within 30 days of application
- (2) passport-sized colour photographs, taken within 30 days of application
- Physician's certificate, issued within 30 days of application
- Letter of employment from Parlour owner
