College of Physicians and Surgeons of Ontario - Doctor Find

  • The public register of the College of Physicians and Surgeons of Ontario includes a wide variety of information, such as each doctor’s name, practice address and telephone number. These are the names of doctors who have advised the college that they are accepting new patients. The public register can be accessed anytime by visiting the website.

Health Care Options:

  • Health Care Connect is an innovative, made-in-Ontario program devoted to helping Ontarians without a family health care provider find one in their community.
  • Ontarians without a family health care provider can call 1-800-445-1822 to register for the Health Care Connect patient database. Ontarians have the option to register through www.ontario.ca/healthcareconnect online.
  • When registering for the program, people will be asked a number of questions about their health needs. The information will then be reviewed by a nurse, known as a Care Connector, from the Erie St. Clair Community Care Access Centre (ESC CCAC).
  • The Care Connector will work with local family health care providers in the community to determine which providers may be accepting new patients. Though Care Connectors will work hard to find a family health care provider for the caller, there's no guarantee that they will find a successful match. As a result, people who sign up for Health Care Connect will also be encouraged to continue their own efforts to find a family health care provider.
  • Telephone services will be available Monday to Friday, 9 a.m. to 5 p.m., in English and French with 120 other languages available, as required.
