Applicant Requirements:
- Application - Holistic Practitioner
- Certificate, diploma or other documentation for Holisitic Practitioner training
- Complementary Health Care Certificate for each modality
- Complementary Health Care Organization Member Certificate
- Code of Ethics (Health Care Organization)
- 2 pieces of government-issued identification - one must have a photo and one must have the applicant's date of birth date
- Copy of advertising, business cards
- Police Records Check (Original-Not more than 3 months old)
- List modalities on licence
- Photo ID card will be issued
