Applicant Requirements:
- Holistic Centre Application
- Business Ownership Declaration-Holistic Centre
- Police Clearance (not more than 3 months old)
- Partnerships- a written declaration signed by all partners stating the I.D. of every partner, I.D.'s under which they intend to carry on business, and mailing address.
- Corporation -copy or notarial copy of letters of incorporation and the most recent annual return including a list of all shareholders of the corporation. If a shareholder is a corporation, the annual return for that corporation is also required.
- Certificate, diploma or other documentation proving that there has been training for the holistic services to be offered (if the owner is offering the services as a practitioner)
- Verification of membership in a complementary health care organization (if the owner is offering the services as a practitioner)
- Code of Ethics (Health Care Organization)
- Photo I.D. to be issued
- Samples of intended advertising material for both practitioners and centre owners
- Certificate of insurance showing at least $2,000,000 minimum liability coverage
- Floor plans (2 sets)
- List of employees and valid municipal licence numbers
- List of modalities to be printed on licence
